Before going any further (by
clicking the link indicated at the bottom of the page to
download the balance transfer application) here are the instructions
for completing the application
accordingly.

Click the link below and the application will download and open up
in a new browser window.
- Fill out the form completely by putting all required information
in its correct place.
- For the boxes that needs to be filled, double click on
the correct box and use the fill option drop down menu that pops up to fill the
box with a desired color (preferably black).
- After you have finished filling out all the required information,
you can simply type in your signature at the bottom of the page with
your desired font style.
- Save the document on your hard drive/desktop by clicking in the
file drop down menu in the upper left hand corner as the name given.
- Email it back to us in an attachment email (in the same word doc format). The title of the email
should be "BALANCE TRANSFER APPLICATION".
