File a Consumer Complaint:
From this page, you can file a complaint directly with your state insurance department.
The states colored in blue on the map below have an online site for filing a consumer
complaint directly with that state. Click on any of these states or the
corresponding state name link below to be taken to that state's online
filing site. To file a complaint with those states that do not have their
own online site for filing consumer complaints, click on any state colored
in yellow or the corresponding state name link below. Then complete the
online form, which will be forwarded directly the appropriate department
in that state to handle your complaint. The green-colored states only
accept complaints mailed as a "hard copy" and do not accept complaints
filed online. Locations colored in red are those that are integrated with the
NAIC State Based Systems (SBS). These complaints will be sent to the states via
SBS after the user has completed the online form. For more information, please
